By accessing or using our website, submitting a form, using the online booking portal, requesting an appointment, or clicking “I Agree,” “Accept,” “Book,” or a similar button, you agree to these Terms. Wise Aesthetics is a self-pay practice. We do not bill commercial insurance, Medicare, Medicaid, or other health plans for our services.

2. Emergency Disclaimer

The website, online booking portal, contact forms, text messaging, email, and other online services are not for emergencies or urgent medical concerns.

If you are experiencing a medical emergency, call 911. If you are experiencing an urgent post-treatment concern, contact Wise Aesthetics directly by phone using the instructions provided to you after treatment or seek appropriate medical care.

Do not use the website, online booking portal, public social media, public reviews, or general contact forms to submit urgent medical concerns.

3. Website Content Is Informational Only

The content on our website, booking portal, Learn articles, treatment descriptions, pricing pages, membership or plan pages, emails, social media, and other online materials is provided for general informational and educational purposes only.

Online content does not constitute medical advice, diagnosis, treatment, or a provider-patient relationship. A provider-patient relationship is formed only when Wise Aesthetics accepts you for care and provides a clinical evaluation or treatment.

You should not rely on website or portal content as a substitute for professional medical judgment, individualized evaluation, or in-person consultation.

4. Online Booking Does Not Guarantee Treatment

Scheduling an appointment does not guarantee that you are a candidate for any treatment.

All medical aesthetic treatments are subject to provider evaluation, medical history review, candidacy, contraindications, informed consent, provider judgment, applicable law, and scope-of-practice requirements. Wise Aesthetics may decline, postpone, modify, or cancel a treatment if a provider determines that treatment is not appropriate, safe, medically indicated, or within Wise Aesthetics’ policies.

Treatment recommendations are made only after evaluation by a licensed provider or qualified professional acting within the appropriate scope of practice.

5. Eligibility to Use Online Booking

You must be at least 18 years old to book an appointment through the online booking portal.

Services for minors may be arranged only with the consent and involvement of a parent or legal guardian and in accordance with Wise Aesthetics’ policies, provider judgment, and applicable law.

By using the online booking portal, you represent that:

• You are at least 18 years old;
• The information you provide is accurate, complete, and current;
• You are booking for yourself or are legally authorized to act for the person receiving care;
• You will update Wise Aesthetics if any information changes before your appointment;
• You will not use another person’s identity, account, payment method, or contact information without authorization.

6. Accuracy of Information You Provide

You agree to provide accurate, complete, and current information when booking an appointment, completing forms, purchasing products, enrolling in a membership or plan, or communicating with Wise Aesthetics.

This may include, where requested:

• Name;
• Date of birth;
• Contact information;
• Medical history;
• Medications;
• Allergies;
• Pregnancy or breastfeeding status;
• Prior treatments or procedures;
• Treatment history;
• Contraindications;
• Relevant health information;
• Payment information;
• Communication preferences.

Failure to provide accurate information may result in cancellation, postponement, refusal of treatment, loss of deposit, cancellation fee, no-show fee, or other action determined appropriate by Wise Aesthetics.

If you arrive for an appointment under the influence of alcohol, recreational drugs, or any substance that may impair consent, judgment, or treatment safety, Wise Aesthetics may decline treatment and charge applicable fees.

7. Self-Pay Practice; No Insurance Billing

Wise Aesthetics is a self-pay medical aesthetics practice.

We do not accept insurance as payment and do not bill commercial insurance, Medicare, Medicaid, or other health plans for our services. You are responsible for payment for all services, treatments, products, memberships, aesthetic care plans, cancellation fees, no-show fees, and other charges you authorize or incur.

Aesthetic treatments are generally elective and are not submitted to insurance by Wise Aesthetics.

8. Pricing, Estimates, and Treatment Costs

Prices listed online, in the booking portal, in marketing materials, in emails, in social media posts, or in other communications are estimates or starting prices unless expressly stated otherwise.

Final pricing may depend on:

• Provider evaluation;
• Treatment area;
• Product selection;
• Number of units, syringes, vials, sessions, or treatment areas;
• Treatment plan;
• Add-ons or combination treatments;
• Membership or aesthetic care plan benefits;
• Promotions, rewards, or credits, if applicable;
• Applicable taxes or fees.

Wise Aesthetics may change pricing at any time. If a price changes before treatment is provided, we will make reasonable efforts to inform you before proceeding.

9. Payment Authorization

By booking an appointment, placing a card on file, purchasing a product, enrolling in a membership or plan, or authorizing a transaction, you authorize Wise Aesthetics or its payment processor to charge your selected payment method for applicable amounts, including:

• Treatment charges;
• Product purchases;
• Membership or plan fees;
• Deposits;
• Cancellation fees;
• No-show fees;
• Late-reschedule fees;
• Applicable taxes;
• Other authorized charges.

Wise Aesthetics uses third-party payment processors. Your payment card information may be collected, processed, tokenized, or stored by the payment processor rather than directly by Wise Aesthetics. Please review the applicable payment processor’s terms and privacy practices.

Do not send credit card, debit card, bank account, or other sensitive payment information through general email, text message, public forms, or social media.

10. Financing

Wise Aesthetics may offer financing options through third-party financing providers.

Financing availability, approval, interest rates, repayment terms, fees, eligibility, and disclosures are controlled by the applicable financing provider. Wise Aesthetics does not guarantee financing approval. Financing agreements are between you and the financing provider.

You are responsible for reviewing and understanding all financing terms before accepting financing.

11. Cancellation, Rescheduling, and No-Show Policy

Wise Aesthetics reserves appointment time specifically for you and your provider.

We require at least 48 hours’ notice to cancel or reschedule an appointment.

Appointments canceled, rescheduled, or missed with less than 48 hours’ notice may be subject to a $100 cancellation or no-show fee.

Appointments booked less than 48 hours in advance may still be subject to the cancellation policy if canceled, rescheduled, or missed.

Wise Aesthetics may waive or modify the fee in its discretion for emergencies, illness, weather, provider availability, or other circumstances. Repeated late cancellations or no-shows may result in a deposit requirement, limited booking privileges, or refusal of future online booking.

12. Deposits and Prepaid Services

Certain appointments, packages, services, events, promotions, or high-demand booking times may require a deposit or prepayment.

Unless otherwise stated in writing:

• Deposits may be applied toward the scheduled service;
• Deposits may be forfeited for late cancellation, late rescheduling, or no-show;
• Prepaid services are subject to Wise Aesthetics’ refund and cancellation policies;
• Prepaid services do not guarantee treatment eligibility;
• If a provider determines that you are not a candidate for the scheduled treatment, Wise Aesthetics will discuss alternative options, credit, or refund handling as appropriate.

13. Refunds, Credits, and Completed Services

Medical aesthetic services involve clinical judgment and individual biological response. Results vary from person to person and cannot be guaranteed.

For services that have been rendered or completed, Wise Aesthetics generally does not offer refunds. If you are dissatisfied with a completed service, contact Wise Aesthetics within 30 days so our team can review your concern and discuss appropriate next steps.

Unused prepaid services, packages, credits, memberships, plans, retail products, and promotional purchases may be governed by separate written policies, purchase terms, membership terms, promotional terms, or refund and cancellation policies. If a separate written policy applies, that policy controls.

14. Retail Products

Retail skincare and other products may be subject to separate return, refund, exchange, manufacturer, brand portal, or ecommerce terms.

Opened, used, prescription, special-order, promotional, or final-sale products may be non-returnable unless otherwise required by law or stated in a written Wise Aesthetics policy.

Wise Aesthetics may later operate a separate retail skincare shop on a subdomain, such as shop.wise-aesthetics.com. Purchases through that shop may be subject to additional ecommerce terms, shipping terms, payment processor terms, manufacturer terms, and product-specific policies.

15. Memberships and Aesthetic Care Plans

Memberships, aesthetic care plans, subscriptions, treatment credits, plan benefits, automatic billing, minimum commitments, cancellation rights, rollover rules, expiration rules, and exclusions are governed by Wise Aesthetics’ separate Membership Terms or Aesthetic Care Plan Terms.

Enrollment in a membership or plan does not guarantee treatment eligibility. Treatment recommendations remain subject to provider evaluation, informed consent, medical appropriateness, provider judgment, and applicable law.

Memberships and aesthetic care plans are intended to support consistency, planning, priority access, predictable budgeting, and long-term care. They are not a guarantee of a particular treatment, result, product, provider, appointment time, or outcome.

16. Promotions, Rewards, and Referral Programs

Promotions, rewards, referral credits, event offers, package offers, limited-time offers, and other special programs may be subject to separate terms, eligibility rules, expiration dates, exclusions, and restrictions.

Wise Aesthetics may modify, suspend, or discontinue any promotion, reward, or referral program at any time, subject to applicable law and any written terms provided for that program.

Referral rewards, if offered, have no cash value unless required by law, are not transferable, may be limited to eligible services or products, and may not be combined with other offers unless expressly stated.

If you receive any benefit, credit, reward, discount, free service, free product, or other value for referring, reviewing, endorsing, posting about, or promoting Wise Aesthetics, you are responsible for clearly and honestly disclosing that connection when required by law or platform policy.

17. Gift Cards

Gift cards, if offered, are subject to separate gift card terms and applicable law.

Gift cards are not redeemable for cash except where required by law. Gift cards may not be used to purchase other gift cards. Lost or stolen gift cards may not be replaceable unless required by law or verified by Wise Aesthetics.

Gift cards do not guarantee treatment eligibility. All treatments remain subject to provider evaluation, candidacy, informed consent, and applicable law.

18. Communications About Appointments and Services

By providing your phone number, email address, or other contact information, you authorize Wise Aesthetics to contact you about non-marketing matters related to your care, appointment, account, forms, scheduling, payment, treatment preparation, treatment follow-up, and related services.

These communications may occur by phone, voicemail, email, text message, booking portal, patient communication platform, or other contact methods you provide.

Transactional and service-related communications may include limited health-related information. You may request confidential communications or alternative communication methods as described in our Notice of Privacy Practices.

If you opt out of transactional text or email communications, you may not receive important appointment reminders, scheduling updates, forms, payment notices, or treatment-related information.

19. Marketing Communications

Wise Aesthetics may send marketing emails, newsletters, educational content, promotions, event invitations, or similar communications where permitted by law and your communication preferences.

Marketing text messages require separate consent. Consent to marketing text messages is not required to book an appointment, purchase a product, receive services, or receive treatment at Wise Aesthetics.

Message and data rates may apply. Message frequency may vary. You may opt out of marketing text messages by replying STOP or following the instructions provided in the message. You may opt out of promotional emails by using the unsubscribe link in the email.

Opting out of marketing communications does not necessarily stop transactional, appointment-related, payment-related, safety, legal, or care-related communications.

20. Privacy Policy and Notice of Privacy Practices

Your use of the website, booking portal, forms, and online services is governed by our Website Privacy Policy.

Health information collected or maintained in connection with your care may be governed by our Notice of Privacy Practices.

By using the website or online booking portal, you acknowledge that you have access to these policies. Your acceptance of these Terms does not replace separate consents, authorizations, treatment consents, financial agreements, membership agreements, Notice of Privacy Practices acknowledgements, or other forms that may be required.

21. Clinical Photos, Marketing Photos, and Testimonials

Wise Aesthetics may take clinical photographs or videos as part of your medical record, treatment documentation, consultation, follow-up, safety review, or health care operations.

Wise Aesthetics will not use your identifiable photographs, videos, before-and-after images, testimonial content, or clinical information for public marketing, advertising, website content, or social media without a separate written authorization or consent as required by law.

Accepting these Terms does not authorize public use of your photos, videos, testimonial, or clinical information.

22. Results Disclaimer

Individual results vary. No specific outcome, result, improvement, duration, timeline, or level of satisfaction is guaranteed.

Medical aesthetic results may depend on many factors, including anatomy, age, skin quality, lifestyle, medical history, product selection, device settings, treatment plan, number of treatments, aftercare, healing response, and maintenance.

Some treatments require a series or ongoing maintenance. Some treatments may involve downtime, discomfort, swelling, bruising, redness, pigmentation changes, adverse effects, complications, or other risks. Risks, benefits, alternatives, and aftercare will be reviewed as part of your treatment process.

23. User Accounts and Portal Access

You are responsible for maintaining the confidentiality of any username, password, login credentials, verification code, or portal access used to access Wise Aesthetics’ online services.

You agree not to share your login credentials with others. You are responsible for activity that occurs through your account, except where caused by Wise Aesthetics or its vendors.

Notify Wise Aesthetics promptly if you believe your account or login credentials have been compromised.

24. Acceptable Use

You agree not to:

• Use the website, portal, or online services for unlawful, fraudulent, abusive, or harmful purposes;
• Submit false, misleading, incomplete, or impersonated information;
• Attempt to access another person’s account or information;

The website, portal, logos, trademarks, service marks, text, graphics, images, videos, design elements, treatment descriptions, educational content, and other materials made available by Wise Aesthetics are owned by Wise Aesthetics or licensed to Wise Aesthetics.

You may use these materials for personal, non-commercial purposes related to learning about Wise Aesthetics or booking services. You may not copy, reproduce, distribute, modify, publish, sell, exploit, or create derivative works from Wise Aesthetics content without prior written permission.

Third-party trademarks, product names, device names, and brand names belong to their respective owners.

26. Feedback and Suggestions

If you provide comments, suggestions, ideas, reviews, survey responses, or other feedback to Wise Aesthetics, you agree that Wise Aesthetics may use that feedback for business, operational, quality-improvement, or marketing purposes, subject to applicable privacy laws and our Notice of Privacy Practices.

Wise Aesthetics will not publicly disclose your protected health information or identifiable clinical information in marketing materials without authorization where required by law.

Third-party services may have their own terms, privacy policies, security practices, and eligibility rules. Wise Aesthetics is not responsible for third-party websites, platforms, tools, financing decisions, payment processor errors, outages, or terms, except where required by law.

28. Website Availability and Changes

Wise Aesthetics may update, modify, suspend, or discontinue all or part of the website, booking portal, online forms, or online services at any time.

We do not guarantee that the website, booking portal, online forms, payment tools, or other online services will be available, uninterrupted, error-free, secure, or compatible with every device, browser, or system.

29. Disclaimers

The website, portal, and online services are provided on an “as is” and “as available” basis.

To the fullest extent permitted by law, Wise Aesthetics disclaims warranties related to the website, portal, online content, and online services, including warranties of merchantability, fitness for a particular purpose, non-infringement, accuracy, availability, uninterrupted operation, and error-free performance.

Nothing in these Terms limits any rights, duties, standards of care, professional obligations, informed-consent requirements, privacy obligations, or liabilities that cannot be limited under applicable law.

30. Limitation of Liability

To the fullest extent permitted by law, Wise Aesthetics and its owners, officers, employees, contractors, providers, vendors, and agents will not be liable for indirect, incidental, consequential, special, exemplary, or punitive damages arising from your use of the website, portal, booking service, online content, or online services.

This limitation does not limit liability that cannot be limited under applicable law, including liability for professional negligence, willful misconduct, healthcare privacy violations, or other claims that cannot legally be waived or limited by these Terms.

31. Indemnification

You agree to indemnify, defend, and hold harmless Wise Aesthetics and its owners, officers, employees, contractors, providers, vendors, and agents from claims, damages, losses, liabilities, costs, and expenses arising out of:

• Your misuse of the website, portal, or online services;
• Your submission of false, misleading, incomplete, or unauthorized information;
• Your violation of these Terms;
• Your violation of applicable law;
• Your infringement or violation of another person’s rights.

This indemnification does not apply to claims caused by Wise Aesthetics’ own professional negligence, willful misconduct, or violations of law.

32. Dispute Resolution; Arbitration; Class Action Waiver

Except where prohibited by law, any dispute arising out of or relating to these Terms, the website, the online booking portal, online services, or online transactions will be resolved by binding individual arbitration rather than in court.

You and Wise Aesthetics waive the right to a jury trial and waive the right to participate in a class action, class arbitration, representative action, private attorney general action, or consolidated proceeding.

The arbitration will be conducted by the American Arbitration Association under its applicable consumer arbitration rules, unless another arbitration provider or process is agreed in writing. The arbitration will take place in Hamilton County, Indiana, unless the parties agree otherwise or the arbitration provider allows a remote process.

You may opt out of this arbitration agreement by sending written notice to Wise Aesthetics at legal@wise-aesthetics.com within 30 days after first accepting these Terms. Your opt-out notice must include your name, phone number, email address, and a clear statement that you opt out of arbitration.

This arbitration section does not prevent either party from bringing an individual claim in small claims court where permitted. It also does not prevent you from filing a complaint with a professional licensing board, the U.S. Department of Health and Human Services Office for Civil Rights, the Indiana Attorney General, or another government agency.

This arbitration section is intended to apply only to disputes that can lawfully be arbitrated. It does not limit claims, rights, or remedies that cannot legally be limited, waived, or arbitrated under applicable law.

33. Governing Law

These Terms are governed by the laws of the State of Indiana, without regard to conflict-of-law principles.

Subject to the arbitration section above, disputes that proceed in court will be brought in state or federal courts located in Indiana, unless applicable law requires otherwise.

34. Changes to These Terms

Wise Aesthetics may update these Terms from time to time. The “Last Updated” date reflects the most recent revision.

The version posted or presented at the time you use the website, online booking portal, or online services will apply to that use, except where a change is required by law or applies to ongoing services by its terms.

Your continued use of the website, online booking portal, or online services after updated Terms are posted means you accept the updated Terms.

35. Severability

If any provision of these Terms is found invalid, unlawful, or unenforceable, the remaining provisions will remain in effect to the fullest extent permitted by law.

36. No Waiver

Failure by Wise Aesthetics to enforce any provision of these Terms does not waive Wise Aesthetics’ right to enforce that provision later.

37. Assignment

You may not assign or transfer your rights or obligations under these Terms without Wise Aesthetics’ prior written consent.

Wise Aesthetics may assign these Terms in connection with a merger, acquisition, sale of assets, restructuring, financing, or other business transaction.

38. Relationship of Documents

These Terms govern your use of the website, online booking portal, and related online services. Additional terms may apply to specific treatments, consents, memberships, aesthetic care plans, promotions, financing, retail purchases, gift cards, or other programs.

If there is a conflict between these Terms and a more specific written consent, treatment agreement, membership agreement, aesthetic care plan agreement, financing agreement, refund policy, cancellation policy, gift card term, promotional term, or other program-specific policy, the more specific document controls for that subject matter.

These Terms do not replace treatment-specific informed consent, financial responsibility forms, Notice of Privacy Practices acknowledgements, photo/video authorizations, or other authorizations Wise Aesthetics may require.

39. Entire Agreement for Website and Online Services

These Terms, together with the Website Privacy Policy, Notice of Privacy Practices, and any applicable written policies or agreements referenced in these Terms, constitute the agreement between you and Wise Aesthetics regarding your use of the website, online booking portal, and related online services.

You also acknowledge that accepting these Terms does not replace any treatment-specific informed consent, financial agreement, membership or aesthetic care plan terms, Notice of Privacy Practices acknowledgement, photo/video authorization, marketing consent, or other authorization that Wise Aesthetics may require.
